- Perio Plan – Up to 4 Perio Maintenance Cleanings, 2 Exams & Necessary X-rays
- 1 Emergency Care Visit: Exam and Necessary X-rays
- Oral Cancer Screening
- Up to 2 Fluoride Treatments When Indicated
